Most of our clients come to Comfort Care with very high level needs and benefit from Comfort Care’s flexible approach, although only a very low percentage of service users have been readmitted to hospital. Over 95 per cent have moved on to live in the community; 7 per cent have moved on or are in the process of moving on, into other services where their needs could be met more appropriately, and only 6 per cent have been readmitted to hospital.
